Survey pages are used to collect feedback from your learners at a designated place within a course.
Use Case Information
A common use case is to create an end-of-course survey that displays as the last page in the course. Surveys can also be given at different points during a course to see how their opinions change over time, for example. Survey questions are not mandatory, unless you require learners to answer questions before they can progress to the next page. There are no correct or incorrect answers.
You might use surveys to gather feedback for your own use or to help learners process their own thoughts through a set of questions that are contained in a no-stakes assessment.
How to Create a Survey Page
- From your homepage, select Courses > All Courses.
- Click the Content Title for to the course you want to alter.
- Within the course outline, go to the lesson where you'd like to add a survey page.
- Click the plus sign in your course outline > hover over the Add Page icon and click.
- Choose the survey page type.
- Add a page title > click Save.
- Click on Create New Question. From here, you have the option to add multiple choice, true/false, image comparison, open ended, file submission, essay, select boxes, table, or Likert scale question types.
Note that there is no question type will have the option to select a correct answer because surveys are not graded.

Multiple Choice: Learners select one or more answer choices in response to a question.

True/False: Learners decide if a statement is true or false.

Image Comparison: Learners decide which one of two images most accurately relates to the question.

Open Ended: Learners provide text a response in an open text field.
You can pre-fill an answer in this question type, which can be used as a foundation to an answer or can simply be used as base text that a learner can manipulate in the response field to add or delete any part of the pre-filled text.
You can also copy previous responses, so if there are previous learner responses in the survey, it will show in this question's answer field. -
File Submission: Learners provide a file submission in response to a question or prompt. Some acceptable file types include PNG, JPG, GIF, PDF, and MP3. Files that contain extensions with executable scripts, like .html, .svg, and .exe, are not allowed to be uploaded. File size limit is 20MB. File submissions are found under the Grading tab of the admin interface.

Essay: Learners submit a text response in a text field (WYSIWYG) in response to given prompts. A Content to Review field can be used to give context or essay instructions. The Question prompt is used define the learner's task.

Select Boxes: Learners select as many boxes as the question requires.

Table: Create prompts for learners to respond to directly in a table. You can dictate row and column headers, as well as the number of columns and rows that are included. You also have the ability to fill in the table fields of your choice and lock individual fields so learners cannot changes the text in a locked table field.

Likert Scale: Use a Likert scale question if you'd like to give the learner the ability to give a response about an attitude or opinion that is typically based on a numerical rating scale. You can add up to 10 rows of questions with the same scale.
Note
For Likert scale questions to function properly, their weights should ideally be set to distinct values representing the scale of responses, such as 1, 2, 3, 4, 5, etc. It is not recommended for all Likert questions having their 'Weight' set to '1' as this can cause the answers to not be recorded as intended.

Multiple Choice: Learners select one or more answer choices in response to a question.
- Click Add Question to save your question to the survey.
- Repeat these steps until all of your questions have been added.
- Next, update your survey options below the questions under Survey Options.

Prevent learner from progressing until survey is answered: Once enabled, a learner must complete the survey in order to continue progressing through the course. The page navigation is grayed out until the learner completes the survey.
Tip
If you need to configure required questions, you can do so after enabling the setting "Prevent learner from progressing until survey is answered?" under Survey Options. Then, edit each question and enable that toggle. Remember to save each question.
- Add an end of survey message: Enter a custom message in the WYSIWYG that will display to the learner on the end screen after responses are submitted.
- Allow instructor to take the survey on behalf of learner: An admin or instructor role can use this setting to take a survey on behalf of a learner through the admin or instructor interface.
- Manually grade survey: An instructor will manually grade the survey.

- Select and save any applicable Advanced Settings:
- Unlocking for editing by children?: This allows any page that is unlocked to be edited by a Client Admin in Panorama. The Client Admin will be able to make changes to the unlocked page in a course in their Panorama only (other Panoramas will not see these changes).
- Add an Audio Narration File: Add an mp3 file here if you would like learners to have the option of playing an audio file on a given page.
- # of seconds required for completion: This settings requires learners to be on this page for the number of seconds you require in order to mark the page as complete. Times is entered in seconds only (e.g. enter 180 seconds to represent three minutes).
- Click Save.
Reporting on Survey Results
Survey results will populate in the Assessment Results Overview report, with the exception of Likert Scale questions with more than one row. If you've built Likert Scale questions with more than one row, those results will populate in the Likert Results Overview report. Notes, Tips, and Considerations
- Question Count: Surveys can have as many questions as desired.
- Text Blocks: When using a survey page, you have the ability to add additional information in text blocks. This can be helpful if you need to separate information, add images, or give directions for a specific question, etc.
- Question Body: The question body is a WYSIWYG, so you are free to add images, HTML, links, and more in order to create a robust question experience. Note that answer choices are not WYSIWYGs.
- Best Practice: Create a test learner so you can sign in as a learner and take the survey to get the full learner experience. Refine your settings based on your experience as a test learner.
